Services: Behavior Services
The Behavior Services department will work in conjunction with your regular veterinarian to provide the comprehensive care your pet needs. Our behaviorist has extensive training in treating a wide range of pet behavior issues.
Behavior Forms
Please fill out the appropriate forms prior to your first visit.
We have created two sets of forms - one group is in word. If you prefer to type you can open these, save, then email, mail or fax them back prior to your appointment.
If you prefer to handwrite your forms, we have created another group of forms with enough space for you to write in your comments.
Before you Start
Before you begin to fill out the necessary forms for you appointment please note the following important (time saving) notes:
- If your appointment is for a behavior issue (& not related to house soiling, marking or furniture scratching): Please fill out the behavior form for either your dog or cat. Read all the way through. If the questions in sections I and II (at the end of the questionnaires) apply to your pet, please fill them out. If not, you can skip those sections.
- If your appointment relates to inappropriate elimination (house soiling/marking): Please ONLY fill out the inappropriate elimination form for your dog or cat. You do not need to fill out the behavior form in this case.
- If your appointment is for a cat who is scratching furniture: Please ONLY fill out that form. You do not need to fill out the behavior form in this case.
